Synchronizing Heiarchy Collections

I'm having an issue when I go to synchronize my hierarchy collections. It will run up to 10% then stop and give me the following failure:

Hierarchy Collections Synchronization for Cost Center hierarchy Failed.

I was wondering if anyone has experienced this and is so what did you do to resolve this. Here is what showed up in the logs:

Error occurred during Asset's folder synchronization. ( Unhandled exception. Type=Altiris.NS.Exceptions.AeXInvalidOperationExce ption Msg=Cannot add an item to a folder that the item already belongs to. Src=Altiris.NS StackTrace= at Altiris.NS.ItemManagement.Folder.Add(IItem item) at Altiris.AssetContractCommon.Settings.SyncHierarchy Collections.UpdateSyncHierarchyCollections(Guid hierarchyType, String hierarchyName) at Altiris.AssetContractCommon.Settings.SyncHierarchy Collections.Process_Execute(GuidCollection organizationResTypes) )

The issue is located in:
Configuration > Soluitions Settings > CMDB Solutions > Synchronize Hierarchy Collections
- the error is during the synchronization of the Departments

Hello,
If I run:
Quote:
Select * from Inv_Department_Details order by _id
I am getting 222 items returned.
If I check on the NS Console Configuration > Soluitions Settings > CMDB Solutions > Synchronize Hierarchy Collections I am getting 433 items.
same as the query:
Quote:
select * from Item where ClassGuid = '9663B667-C60D-4246-81C8-0F7E09587384'
If I run the following query:
Quote:
select * from Item where ClassGuid = '9663B667-C60D-4246-81C8-0F7E09587384'
where state like '%item%'
the %item% is showing the old server name with a url!!!

<item> <sourceNS guid="{f9e2b418-8b4e-4df9-a494-b2a747d2328d}" name="oldserver.domainname" url="http://oldserver.domainname/Altiris/" /> </item>
This does not refer to the current NS? it is the same guid (select * from vThisNS) as the Database was restored but the name has changed, should this information been changed also in all tables????

I am getting the 121 items difference between the Inv_Department_Details table and the Item Table.
What does this mean?
